Yeah, he is further turned upwards and his left arm is more vertical than in other photos I've seen of him. This is definitely his intention.

I could tell because his eyes are looking up and sort of behind him. This indicates that the ball is traveling behind his head which is where you would place the ball, for a topspin serve.

Yes, it is the angle of his back and shoulders closed off for the cartwheel. I think his second serve is safer for the lower back than the Sampras serve, because the bow shaped arch is not as extreme. The right shoulder and elbow does not keep going back towards the baseline, and upon rotating forward, does not pinch the the lower back and wear the left hip socket.
