You're using too conservative a grip. I don't see much racquet face inversion. Also, you're not holding the position long enough to create a slot. It is not a helicopter swing.

Your stance keeps changing and your weight transfer is circular. Not correct. Try to keep 45 degrees first every time with the feet. The weight transfer is a dip and then a turn but the weight goes to the left foot. 

You are launching the ball erratically because you haven't pointed the buttcap and done enough racquet drag. Then you rotate too quickly and the bottom edge points up. Close the face and drag the head.
